[Instructions for filling out the Claim Form: Please digitally fill the form, then print it, sign it, attach ALL your original receipts (feel free to make a scan/copy of your receipts beforehand for your own records), and also print out the website page(s) that you use to make any currency conversions. Once you have filled out the form, please email it to me so that I can check it. It's not an intuitive form to fill out and if there are any mistakes, etc, it's best for me to spot them before you go to the trouble of mailing it to the UK!
Then, once your form and receipts are complete, please mail everything to:

Alternatively, instead of mailing the form, you can simply hand it (and your receipts) into me in person when you come to Oxford in October (although this means that any receipts you incur on your return journey - e.g. taxi home from the airport, will have to be claimed later by filling out another form). Whatever you do, please do not mail the form to the address that is listed on the form - it might get lost in university bureaucracy.]
